Accepted

  • Handwritten prescription with physician signature
  • Electronic or e-prescription printout
  • Pharmacy printout with prescriber details
  • Telehealth prescription with valid NPI number
  • Prescriptions issued within the last 12 months

Not Accepted

  • Expired prescriptions (older than 12 months)
  • Prescriptions without physician signature
  • Altered or modified prescriptions
  • Illegible prescriptions
  • Prescriptions from unlicensed practitioners
